RACHAEL JULIA COOK (of Lane End) Passed away peacefully on Wednesday 19th April 2017 aged 84. She was much loved by her children, Stuart, Caroline, Iain, Jane and Niall and by her grandchildren, great grandchildren and friends. Funeral to be held in The Milton Chapel, Chilterns Crematorium, Amersham on Thursday 11th May at 10:45 am followed by Memorial service at St John the Evangelist Church, Frieth at 1:00 pm. Family flowers only please. Donations in lieu of flowers to Cancer Research UK and British Heart Foundation c/o Surman & Horwood Funeral Service.
